Download and fill in a copy of the official Advanced Care Plan to ensure your requests are heard for the end of your life. You can prevent any chance of assisted suicide or euthanasia being a threat to your future now by making your requests known and legally binding.

Advance care planning is the process of thinking about, talking about, and planning for future health care and end-of-life care. It usually involves the individual, the health care professionals responsible for their care, and a person’s family/whanau and/or carers. 

While the end of life may be a challenging thing to confront, by making some decisions now you are securing your future.
